Proof of Residency
Proof of residency is required for students enrolling in courses at FLVS or who are participating in the Florida Dual Enrollment Program. Your documents may be mailed (recommended), emailed, or sent to us via our online submission form. In addition to providing proof of residency to us, you will need to provide proof of residency to FLVS and/or the college providing dual enrollment.
As email is not a secure medium, we discourage you from emailing your documents to us. If you choose to email them, note that we do not accept photos taken with a mobile device. Please use a flatbed or sheet fed scanner if possible. If you use a scanning app, please correct any perspective and lighting issues and crop them before saving. Residency is verified annually or as needed.
Acceptable Documentation
We accept copies of the following documents for proof of residency. We will need one document from Group A and one document from Group B. Your documents must be current and be in the name of the registering parent or guardian. The address must be a residential address and NOT a post office box, third party mailbox, or commercial address. The name and the address on both of your documents must match. You are welcome to block out sensitive information such as account numbers and account balances, but your documents must show the name and address of the issuer, the date the document was issued, and your name and residential address. To protect your identity, we recommend you mail your documents.

Families Living with Other Families
If your family resides with another person or family, we will need the following:
- A signed Residency Affidavit and
- One document from Group A from the homeowner or lessor and
- One document from Group B from the parent or guardian.
Military Families
If your family is living outside of Florida due to military service, we will need the following:
- Military Orders and
- DD Form 2058, State of Legal Residence Certificate or DFAS Form 702, Defense Finance and Accounting Service Leave and Earnings Statement
