You may use this form to send us your documents, but you may send them to us by mail or email as well. Please do not send screen shots or photos taken with a mobile device. If you use a scanning app, please correct any perspective and lighting issues and crop them before saving. Please use a flatbed or sheet-fed scanner if possible. Please mail your documents if you are not able to scan them.

The information and documents you submit with this form will be forwarded to us via TLS encryption. Your data will not be stored online.
