Should you move out of the state of Florida or otherwise wish to withdraw your student from Aspiring Heights Academy, we ask that you please notify us in writing. You may use the form below or notify us via mail or email. We will need your student's name, date of birth, reason for withdrawal, attendance record, and a records release if you would like us to send our records to your student's new school. You're welcome to include any other academic records that you would like us to keep in your student's permanent school file. Please do not send screen shots or photos taken with a mobile device.
The information and documents you submit with this form will be forwarded to us via TLS encryption. Your data will not be stored online.
If your student's new school needs us to verify that your student has been withdrawn, or if you would like us to send our records to your student's new school if/when requested, please include a signed release.
