Credit by Portfolio Packet

This packet includes forms for documenting high school coursework for students in our High School Diploma Program. Credit is subject to approval. The Course Information form is completed and signed by the parent. The Instruction/Activity Record is completed by the parent or student. The Credit Verification form is completed and signed by a Florida certified teacher. Instructions are included.
